I would think this is a single activation---the Defending unit get First fire on all the units attacking it during the activation (if entrenched or dug-in--and any other attacks
later).
The original situation, so far as I can recall from my question on Consim, had an infantry platoon (maybe 2) with a captain, adjacent to enemy infantry. Behind them was a cavalry troop with a lieutenant (cavalry leader). The captain activates himself and his infantry to move to assault (back then assault was a move order) and the LT who tells his troop to charge.
